How the Process Works

This form helps me understand your needs and determine appropriate fees based on your situation.
I use Anti-Racist & Anti-Oppressive Fee Rates, considering socioeconomic factors to make my services accessible.
You'll share information about your accommodation needs, challenges, and financial circumstances. This helps us determine if I'm the right fit and allows me to prepare for our intake session.
I offer payment plans and sliding scale options based on your information, aiming to make accommodation support accessible while maintaining a sustainable practice.
Schedule Intake Appointment
Begin by scheduling an intake appointment with me. During this session, I'll learn about your specific needs, your environment (school, workplace, or other), and the accommodations you're seeking. You'll complete some screeners during our meeting, with additional ones to finish at home if necessary.
Drafting Your Letter
Using your intake form and screeners, I’ll create a personalized accommodation letter that clearly and respectfully outlines your needs using professional and supportive language.
Review & Revisions
I’ll send you the draft within X business days (insert your timeline). You’ll have the chance to give feedback, and I’ll revise it to ensure it feels like your voice is being represented.
Final Delivery & Guidance
Once complete, you’ll receive your finalized letter along with a simple “next steps” guide to help you as you submit it to your institution or workplace.
A Note on Outcomes
I want to be transparent: I cannot guarantee that your accommodations will be accepted by your school, employer, or institution. Every organization has its own policies and decision-making processes. I provide accommodation letters for school (including testing accommodations) and work environments only.

Emotional Support Animals (ESAs) - These require specific clinical documentation that falls outside the scope of my services.
Housing Accommodations - Housing accommodation requests follow different regulatory frameworks and documentation requirements that are not part of my current practice.
If you need documentation for ESAs or housing accommodations, I recommend consulting with a provider who specializes in these specific areas.
